I lived in Georgia at the time of TM onset, and I got the form for a temporary
handicapped placard from the neurologist's office. It was filled out there and
my husband took it to the DMV. I can't remember for sure, but I don't think it
cost anything. It had to be renewd every six months until I felt I could do
without it. The DMV was VERY particular about the form being filled out
exactly...the first time, something was written on the wrong line, and
although all the pertinent info was there, we still had to re-do it.
